Urgent advice: Urgent Same Day Appointments

These are available via the phone from 8.30am for problems which require attention that day.

Phone us on 0114 232 2340 (Dykes Hall), 0114 234 7979 (Deer Park), Monday to Friday from 8.30am to 6:00pm

Visit the surgery and speak with a receptionist, Monday to Friday from 8.00am to 6:30pm

Examples: symptoms of chest infection, urinary tract infection, ear infections, skin infections. Urgent mental health problems. (Mental health crisis can call 111) Patient concerned about cancer, for example breast lumps, worrying bleeding, Palliative care.

Once the appointments are full, you may be advised to attend the walk in centre so that your urgent problem can be dealt with that day. Our receptionists can access bookable same day appointments at the local hub subject to availability.

Please consider seeing the community pharmacist for simple conditions such as ear infections, impetigo, infected insect bites, shingles, sinusitis, sore throats, urinary tract infection in women aged 16-64.

Receptionists will consult with the GP where these options are not appropriate for the patient.

 

Non-urgent advice: Routine Appointment

For routine appointment requests which can wait up to 2 - 3 weeks please follow the link below to complete and submit a triage form.

Online Consultation Form (AccuRx)

This will be triaged by reception or where needed by the GP. Where an appointment is needed, a booking link will be sent for you to book an appointment within 48 hours. A choice of clinicians will be offered. You may have to wait slightly longer for the clinician of your choice.

Examples: less urgent mental health issues, chronic conditions, medication reviews, follow up appointments.

For Nurse and HCA Appointments Please continue to ring the surgery rather than using the form. 

 

Non-urgent advice: Your Appointment

Please tell us:

  • If there’s a specific doctor, nurse or other health professional you would prefer to respond.
  • If you would prefer to consult with the doctor or nurse by phone, face-to-face.
  • Appointments by phone can be more flexible.
  • If you need an interpreter.
  • If you have any other access or communication needs.

 

Non-urgent advice: Cancel or Amend an Appointment

To cancel or amend your appointment, you can do the following:

  • Use your NHS account (through the NHS website or NHS App).
  • Use systmone online account.
  • Phone reception on 0114 232 2340 (Dykes Hall) or 0114 234 7979 (Deer Park), Monday to Friday from 8.30am to 6:00pm.
  • Visit the sugery and speak to a receptionist .

Non-urgent advice: Home Visits

If you are housebound and need an appointment, we will visit.

Requests for home visits should be made only if it is impossible for the patient to get to surgery. 

Please ring before 10.00am and give as much information as possible to the receptionist.

Only urgent calls should be requested after 10.00am.

A doctor will then phone you first to either advise or organise a home visit.

Non-urgent advice: Patient Responsibilities

Keeping mobile numbers up-to-date. If we have your current mobile number on your medical records you will receive a sms reminding you of your appointment

Arriving on time. We understand how irritating it is to have to wait. Our clinicians try their best to work to their appointment times and we hope you do too. If you arrive late for an appointment you may be asked to make a new appointment.

Cancellations. If you are unable to make your appointment then please cancel it as early as possible.

Non-urgent advice: Enhanced Access Service

The Enhanced Access Service has more appointments with the GPs, Nurses & Health Care Assistants available, making it easier for you to get an appointment at a time that suits you, including evenings and weekends.

Booking an Appointment

Contact your GP practice in the usual way to request an appointment. If there is no appointment available at your practice, they may offer you an appointment with the GP Enhanced Access Service, if this is right for you.

Please note that your consent will be required in order for an appointment to be booked for you.

Appointments available every week

  • Mondays,Tuesdays & Thursdays 7.00am to 8.00am – Health care assistant appointments available
  • Mondays 6.30pm to 8.30pm – Doctors & Nurse appointments available
  • Saturdays 9.00am to 1.00pm – Doctors appointments available at one of the 4 sites (Dykes Hall, Tramways, Fairlawns, Far Lane)

Appointments available 1 in every 4 weeks

  • Fridays 6.30pm to 8.00pm – Telephone appointments available with a doctor
  • Saturdays 1.00pm to 5.00pm - Telephone appointments available with a doctor

Non-urgent advice: Local Pharmacy

As qualified healthcare professionals, pharmacists can offer advice on minor illnesses such as:

  • Coughs
  • Colds
  • Sore throats
  • Tummy trouble
  • Aches and pains

They can also advise on medicine that you can buy without a prescription.

Many pharmacies are open until late and at weekends. You do not need an appointment.

Most pharmacies have a private consultation room where you can discuss issues with pharmacy staff without being overheard.
