Changing your Contact Details

To update details like your name, address, phone numbers or email address, please fill out a change of contact details form alternatively you may phone or visit the surgery during opening hours.

If you have moved to a new address and which to stay registered with our surgery, you will have to ensure you are still within our catchment area. 

If your new address is outside our catchment area, you will need to register at a surgery more local. 

You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
