New Patient Registration

To register with the practice, please complete and sign a registration form as well as completing:

  • A New patient questionnaire
  • Consent forms for electronic recording sharing, and shared care summary
  • Alcohol Questionnaire

These forms can be collected from the reception desk or downloaded & printed from the above “Registering Online” tab. If you are unable to come into the surgery we can post the forms out to you.  

When you return your forms to the surgery you will need to bring a form of photographic evidence for the receptionist to view. You will need to see a doctor if you take regular medication.

The practice has suitable access for disabled patients. We also have a loop hearing device for the hard of hearing; please ask at reception for details.
